Please print a submission form per sample

Ensure each sample is clearly labeled with date of collection, owner and horse name and a submission form attached to each sample.

Samples should be as fresh as possible to ensure accurate result and stored in the fridge for no more than 3 days.

FEC Samples: 1 sample $33                          2 or more samples $30 per sample

PAYMENT:

Invoice will be emailed. Payment is due before sample will be processed and results provided.
